When you schedule your appointment you will be given a check-in time. Failure to arrive at your check-in time may result in your appointment being canceled, re-scheduled or you may have an extended wait.
Once you have checked in, your chart will be prepared and you will meet with the medical support staff to get an extensive medical history, following that you will meet with your dermatology practitioner. Upon completion of your visit with your practitioner, the receptionist will check you out and make further appointments if necessary.

Uptown Dermatology and SkinSpa has contracted with all major third party payers representing the majority of patient health insurance plans in the market. This includes:

In addition, Uptown Dermatology and SkinSpa, P.A. will consider accepting the multitude of private insurance and indemnity plans that are available to patients in the market area. Some of the private/indemnity and third party administrator examples include, but are not limited to, American Republic, Bankers Life, Benefits Inc, CC systems, CIGNA, CNA Insurance, Continental General Corestar, Crawford and Company, Equicor, Farmers Insurance, Federated, Firemans Fund, Galaxy, GEHA, General American, Great West, Liberty Mutual, Mutual of Omaha, Physician's Mutual, Safeco, State Farm Insurance, State Fund Mutual, Wausau Insurance Company, Wilson Shane and World Insurance.

YOUR FIRST VISIT
Plan on arriving to our clinic at your scheduled check-in time. There is a paid parking lot behind our building and can be accessed on Emerson Ave off of Lake Street. We discount the first $1.50 for parking. There are also parking meters in front of our building on Lake Street.
If you have not already filled out the registration form on this website, you will be asked to do so at that time. You will need your insurance identification card as we will make a copy of it for your chart and for the business office. Payment for your visit will be due in full upon checkout if no insurance card is available.
Once your chart is prepared, one of our medical support staff will come and take you to one of our exam rooms and get a detailed history of your condition and prepare you to see one of our dermatology practitioners.
Upon completion of your visit with your practitioner, the receptionist will check you out and make further appointments if necessary.
Your total time spent will be approximately 1 hour from check-in until the receptionist checks you out and validates the first $1.50 of your visit.
Please note: We appreciate that your time is valuable. We will make every effort to see you in a timely fashion. While timeliness is important, the quality of your health care is paramount. Our dermatology practitioners will not sacrifice the quality of your health care just to be on time. To assist us in our endeavor of being timely, we request that you limit your discussion to ONE problem. We will be happy to arrange another appointment if there are more issues to be discussed.
